We are pleased to announce the opening of the Activate Health Center for members of Seymour Community Schools’
insurance program. We are designed to provide full primary care service for SCS
insured employees and family members age 3 or older.
From the moment you enter the Center, you will notice the
focus upon wellness and personalized medical care. The service providers are
trained professionals with years of experience in helping patients take an
active role in their health, quality of life, and overall well-being. Research
has proven this approach to medical care can reduce or even eliminate the onset
of many conditions that present themselves at later stages of life.
Even more exciting is our incentive program. Choose our Health
Center for your annual physical and you will receive a $100 incentive. Work with
our health care providers to set goals and if you reach them you will receive
another $100 incentive. These incentives will be mailed directly to you at home.
We anticipate the Center to open in late August. Stay tuned to

will be the new primary care Nurse Practitioner for the Activate Health Center. I have been practicing in this part of the state for many years. My experience includes:

I left my most recent position to join the Activate Health Center because I believe deeply in
the kind of health care that will be offered to the insured employees of Seymour
Community Schools. The Activate Health Center provides full primary care
services at no out-of-pocket cost to you. Because the Activate
Health Center is funded by your insurance program, we will take care of you
without the usual limitations experienced by most medical providers. We
will spend the time with you that you need. We will help you develop
individually tailored goals and action plans. We will support and coach
you as you work on achieving your goals. We will also save you money by eliminating co-pays for
primary care received at the clinic, providing a limited set of generic drugs at
our facility with no out-of-pocket cost, and gaining access to low cost lab
services. The Seymour Community Schools new incentive plan will pay you and your
spouse for completing an annual physical and making progress toward your
individually tailored goals.
